I’m afraid I forgot the LUKS disk encryption password on my Librem 14 with Qubes OS and the Librem key. 
So, I plugin the key, and it boots until the disk password screen where I’m stuck.
Could someone please point me to instructions on how to do a factory a factory reset or reinstall.
In a related note I remember there were initial instructions from purism, including the default PINs and PUKs I don’t find them. could someone please point me to those?

Factory reset? You might have to re-install with qubes-os.org or pureos.net instructions. New versions are always available. Documentation looks updated. Building memory bridges and rebooting routinely have helped with remembering.
There’s a lot to unpack there, lots of options. I think you would be best to contact Purism Support support@puri.sm before doing too much more.
