Has anyone else started getting this? Never used to do it, but I’m now finding I can make a call, but then the next outbound call rings then craps out. I turn the modem off and on again, then I can make the call fine.
No, what modem firmware and region are you using?

Does the issue still occur after restarting the phone?
I’m in the USA and using T-Mobile. I have not found calling reliable enough to use as a daily driver. I won’t be able to give any concrete and actionable information but the summary is below. For me I use my cell for work and do tech support for our customers. Many times while driving.
- Outbound calls randomly don’t work requiring a reboot to get them back. Many times even cycling the modem switch isn’t enough. Inbound calls still tend to work.
- Headset profile in my car doesn’t work. Its illegal to not use hands free in the car here. This alone should stop me from even trying.
- When on a call if another call comes in I get disconnected. This is repeatable and kinda a big deal.
- Frequently (1 out of 3?) the person I am calling can’t hear my audio.
- Texts seem to get hung up on rare occasion. It seems that it always has to do with multimedia.
Every other week or so I put the sim card back in and try again only to get frustrated and go back to my OnePlus 6 running the deGoogled /e/. My last attempt ended on Monday this week and only lasted 2 days. I was on a long support call and kept getting disconnected by others trying to reach me.
I have been tempted to order the AweSIM plan through purism just to find out if all my issues have to do with trying to use T-Mobile. Perhaps others that use it could comment.
I love my Librem5 and tend to carry it around and use it for everything else I do with a phone. (ssh to our servers, emails, calendar, matrix chat, web browsing, etc) I love that it has a OpenPGP card which I use for authentication to our Linux servers. I use my Librem5 every day and would not want to be without it. I look forward to a day when calls, texts, bluetooth work good enough I can use it 100%.
Where do I pay money again for specific use-cases such as a working modem that makes reliable calls and texts?

I keep chucking them £20 and putting calls as the app. I’ve had all the above issues variously. This one seems to be new though.
I too am on T-Mobile currently in the US. I have submitted my request to Purism to switch to AweSIM so that change is still pending. I haven’t experienced the voice calling issues you have but then again I don’t use voice much. SMS sometimes aren’t delivered or are delivered very late (for unknown reasons), but by flushing texts from the modem, the SMS messages then get through. I’m switching to get away from sale of data by T-Mobile to others (even though I configured my account to opt out of these sales, I don’t trust T-Mobile). I will get back to you about the AweSIM service.
I had similar problems for about six months until the first of the year when I could no longer send or receive any calls or messages. My reseller told me that AT&T no longer supported the Librem5. I switched to the Purism SIM and have had no problems since, other than receiving several unwanted political SMS messages daily.
I think you should be complaining at Purism about that i.e. if you are using SIMple, SIMple Plus or AweSIM. However it may be that they are unable to stop the spammers.
